Here is a link. It's expensive, but it would be one Chelsea jersey personalized, but shirt is always a goalkeeper.

Is all you have to do is buy a T-shirt from a shop owner then football to a place where the print screen. You can sit with them and explain what you want on your T-shirt. That is what my university is not it cost a damn lot.
I agree with Spud. It is much cheaper and you get more input if you do so locally. Google hot press or screen printing + your city.
